TOURNAMENT REGISTRATION FORM

Required documents include a USA team registration card, a completed and signed USA waiver roster, proof of team insurance, and a check for payment. $100 deposit holds your spot; balance is due no later than three weeks prior to start of tournament. This deposit is not refundable, so be certain prior to sending payment.

At this time, USA softball covid papers are required to signed and dated by parents or guardians of athletes participating in this tournament.
Online payments have been enabled to streamline your entry process. If you choose to not use stack pay, please follow information below.
