LEASE INFORMATION
I recognize, acknowledge, accept, and assume the potential risks of injuries and dangers involved in participation in the sport of football and related activities. I recognize and acknowledge that no football equipment, regardless of condition or rating, can fully prevent injuries such as but not limited to concussions. If I choose to provide my child with his or her own helmet, I fully release Jackson Freedom League Youth Football and its board of directors, its officers, members, and volunteers from all liability or responsibility for injury caused by equipment I provide. I have read and accept the warning label placed upon the equipment
and accept the equipment ?as-is?. I acknowledge and agree that Jackson Freedom League Youth Football, its board members, and/or volunteers should not assume responsibility for, nor guarantee the personal safety of participants, nor does Jackson Freedom League Youth Football warranty, guarantee, or assume responsibility for the borrowed equipment.
Further, I hereby release, waive, covenant not to sue, and agree to hold harmless and indemnify the Jackson Freedom League Youth Football board of directors, its coaches, members, and volunteers from any and all liability, damages, claims, demands, or causes of action of any kind and/or nature of which may arise from the use of the equipment borrowed/rented from Jackson Freedom League Youth Football. I further acknowledge that we have carefully read the foregoing release and acknowledgement and know the contents thereof and sign this release and acknowledgement as a free and voluntary act.
