WAIVER INFORMATION

I, the undersigned player/parent/guardian of the above named applicant, do hereby consent to he/she participating in the Rowlett Hockey League, during the current season.

I further agree to release the Rowlett Hockey Assocation, operating as the Rowlett Hockey League, its officers, coaches, and board members, and any or all team sponsors, from liabilities due to injuries incurred by said applicant during the current season.

I am also aware that, although rare, hockey pucks do sometimes leave the playing surface and can and will enter the spectator area, parking lot, or other non designated playing areas. I hereby release the Rowlett Hockey Association, its officers, coaches, board members, and all others from liability of damages to vehicles or injury to spectators or family members of players caused by flying objects that leave the playing surface.

Furthermore, I agree to abide by all rules and regulations adopted by the Rowlett Hockey League during the season.
