If the player is less than 18 years old:
In consideration of permitting above named child (minor)/player to participate in games, practices, and activities of THE ONTARIO CARDINALS, I, the parent/guardian of said child (minor)/player, do hereby release and agree to hold harmless THE TOWN OF AJAX and THE ONTARIO CARDINALS (baseball coaches and volunteers) from any liability for personal injury or property damage which may occur to said child (minor)/player, as a result of his/her participation or as a result of negligence on the part of said program (baseball coaches or volunteers) related to the program.
