Your 2 checks can be mailed to our treasurer, the address will be given at the completion of form. 2024 Parental Consent and Release Form, Swimmer's Code of Conduct, & Parent Code of Conduct Form (Form 2 of 2) Signing this form indicates that you are giving permission for your child to participate as a member of the Lower Macungie Township Lazers Swim Team. I consent to accept and assume such risks and hereby waive any claim against the Township or its supervisors, employees or agents for any injury suffered by my child while on Township property in connection with his/her participation as a member of the swim team or while attending an away swim meet as a member of the swim team. Swimmers and Parents will follow all COVID protocols put into place for the protection of athletes, coaches, and families. Swimmers and Parents will abide by the Code of Conduct at all times while representing the Lazers. Please make sure to finalize your registration by clicking submit and check out at the end of the forms. The mailing address and fees will come up after you complete your registration form. |
