I beleive FIFA should introduce a 30 second time out for video evidence that can be called for by either the manager on the bench or team captain by signaling to the 4th official or referee immediately following a controversial decision (and no more than twice in any game by either team) with a yellow card being given to any team captain for consecutive failed challenges to the video referee. Any thoughts?

How would you signal the official, run towards him with your arm straight up in the air....you'd get two yellows after five minutes in every game:)
