(1) COASTAL CUDAS CODE OF CONDUCT (PRINT FOR REGISTRATION)
Print out this form and please bring it with you on the first day of practice. Please read it carefully and sign it and ONLY return bottom portion and you keep the top.

(2) MEDICAL CONSENT FORM (PRINT FOR REGISTRATION)
Medical release/consent must be filled out with registration to be able to practice without pads!

(3) MEDICAL CLEARANCE FORMS (PHYSICAL/WELL-VISIT: PRINT FOR REGISTRATION)
MEDICAL CLEARANCE FORMS MUST BE FILLED OUT BY A DOCTOR AND RETURNED PRIOR TO BE ALLOWED TO PARTICIPATE IN FULL PLAYER ON PLAYER CONTACT.

(4) CONCUSSION ACKNOWLEDGEMENT FORM (PRINT FOR REGISTRATION)
