 Registration Requirements:

Completed Application (Signed by parent/guardian)

Recent Photo (2x2 Passport preferred)

Proof of Age: (legible copy of birth certificate)  

Registration Fees:

Returning Players: $140 until 6/1/2019

New Players: $150 until 6/1/2019

Returning and New Players: $160 after 6/1/2019

Payment method: (Cash, cashiers Check, or Money order ONLY)

It's imperative that your child accompany you during the registration process to be officially weigh in and place on the age/weight appropriate team.

           ALL PLAYERS MUST BE PRESENT AT THE REGISTRATION PROCESS TO BE WEIGHED IN.

Line* indicates Linemen Weight, 5 players maximum per side must be on the line within 3 yards from the ball and wear a 2" dia. sticker on back of helmet.

Cut-off Date is August 1st.  A player born On or After cut-off date can play down.

PRACTICE SCHEDULE:

·       Practice is Monday thru Friday 6:30 to 8 PM

         Practice dates my change depending upon your team. Coaches will communicate that change if applicable.

 Do Not ASSUME practices is cancelled because of the rain; If the decision is made to cancel practice you will receive a text notification.

No need to call/text prior to 6PM for Practice cancelation inquiries.

       Parents are NOT allowed on the field in close range of the players during practice (to limit distractions):

             NO FRIENDS     NO RELATIVES   NO PETS

If there is an issue or communication bearer with the Coach and Parent, please address those issues before and after practice. Coaches and Parents need to speak away from under age children and in a professional manner.

•      Parents and coaches should always lead by example. Please govern yourself accordingly.
•      Parents are not allowed to interrupt Football practice (Please see code of conduct)

GAME SCHEDULE:

·       Game Schedules from the league are tentative and subject to change. Schedule finalization for each week will be provided to all parents by Wednesday, at the latest prior to the game schedule date

GAME APPAREL:

Hialeah Youth Athletic Club will provide the following equipment/ Apparel to each Football participant that is included in your Registration Fee:

•            Helmet / Face Mask
•            Shoulder Pads
•            Game Uniform Jersey / Pants

HELMETS/ SHOULDER PADS / GAME JERSEYS/ PANTS ARE THE PROPERTY OF HIALEAH YOUTH ATHLETIC CLUB AND MAY NOT BE ALTERED IN ANY WAY. A $50 RETAINER FEE FOR GAME JERSEYS / PANTS WILL BE COLLECTED IF ALTERED IN ANYWAY.

Equipment pick up will occur during the 2-3 week of practices. Equipment is due back immediately after the last game, playoff or Super Bowl game. NO EXCEPTIONS.

(If a player quits, the parents/guardian are responsible for turning in their equipment/ uniforms immediately)

THERE WILL BE A $250.00 FEE BILLED TO YOU IF THE EQUIPMENT IS NOT RETURNED ON THOSE DATES AS PER THE CONTRACT.

PARENT ADDITIONAL PURCHASES:

Players will need to purchase the following items:

•       7 Piece pads (all players must where knee, thigh, hip and tailbone pads) or Girdle w/knee pad
•       2 mouth guards/piece
•       Chinstraps
•       Football cleats/shoes-no metal spikes allowed
•       Practice Pants (Available for purchase at Park)
•       Practice Jersey (Available for purchase at Park)
•  

       Optional Pads: Forearm/hand/elbow and rib pads are available and can be used at the Discretion of the player/parent/legal guardian
