To nominate any current or former player to the Urbandale Sports Association Softball Hall of Fame, please complete an application and submit it to the address provided. All nominee's will be reviewed by the Urbandale Sports Association Board of Directors and voted upon for approval. All approved nominees will be added to the Hall of Fame list above as well as on a plaque located at Walker Johnston Softball Complex. Additionally, new members will receive a letter of congratulations and a certificate of membership to the Hall of Fame.
