Double Diamond Sports Academy Lesson Interest Form


In consideration of the acceptance of myself and/or my minor child to participate in the instruction/training activities of DOUBLE DIAMOND SPORTS ACADEMY, I and/or my minor child hereby WAIVE, RELEASE AND DISCHARGE any and all claims for death, personal injury or property damage which I and/or my minor child, or the heirs, distributes, guardians, legal representatives, or assigns of myself and/or my minor, may have, or which may hereafter accrue, as a result of participation of activities or programs of DOUBLE DIAMOND SPORTS ACADEMY. The RELEASE is intended to discharge in advance DOUBLE DIAMOND SPORTS ACADEMY, any of it’s employees, owners, officers, agents and any and all of the sponsors of it’s activities, from and against any all liability arising out of or connected in any way with participation by myself and/or my minor child in said activities, even though that liability may arise out of negligence or carelessness on the part of the person entities mentioned above.

I further understand tat accidents, whether of serious nature or of minor nature, occasionally occur during the course of baseball instruction/training activities and that participates may occasionally sustain mortal or serious personal injuries, and/or property damage, as a consequence thereof. Knowing the risk of the activities, nevertheless, I hereby ASSUME those risks and RELEASE AND HOLD HARMLESS all of the persons and/or entities above who, through negligence and/or carelessness, might otherwise be liable to me and/or my minor child for damages.

It is further understood and agreed that the WAIVER, RELEASE AND ASSUMPTION OF RISK it to be binding on the heirs and assigns of myself and/or my minor child, and that I and/or my minor child, and the heirs, distributes, guardians, legal representatives and assigns of myself and/or my minor child, will not make a claim against, sue, attach the property of, or prosecute DOUBLE DIAMOND SPORTS ACADEMY, any of it’s employees, owners, officers, agents or sponsors of any ACADEMY activities, for injury of damage resulting from the negligence or other acts, howsoever caused, by any employee, owner, officer, or agent of DOUBLE DIAMOND SPORTS ACADEMY, or any sponsor of ACADEMY activities, as a result of my and/or minor child’s participation in DOUBLE DIAMOND SPORTS ACADEMY activities or programs.

I hereby agree to INDEMNIFY AND HOLD HARMLESS DOUBLE DIAMOND SPORTS ACADEMY it’s employees, officers, owners, agents and the sponsors of the ACADEMY activities, from any loss, liability, claim, damage or cost accrued due to participation of myself and/or minor child in DOUBLE DIAMOND SPORTS ACADEMY activities and programs or due to the presence of myself and/or my minor child in the site of ACADEMY programs or activities, and irrespective of whether loss, liability, claim, damage or cost may have been caused by the negligence, carelessness or other act or omission by DOUBLE DIAMOND SPORTS ACADEMY, it’s employees, owners, officers, agents, or the sponsors of the ACADEMY activities.

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND AND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY, AN ASSUMPTION OF RISK, AND A CONTRACT BETWEEN MYSELF, MY CHILD AND DOUBLE DIAMOND SPORTS ACADEMY. UNDERSTANDING ALL OF THE ABOVE, I SIGN IT OF MY OWN FREE WILL.
