slap hitter goes straight forward towards pitcher, does not swing or offer, but is hit by pitch, out of box because beyond the four foot area from middle of plate. what is the correct call?

Based on the information given, I would call it a ball, and dead immediately.

July 2010 plays and clarifications. If the batter prevented the ball from entering the strike zone, it is a dead ball strike. If the batter attempted to avoid or did not have time to make an attempt to avoid it is a hit by pitch and award 1st base. If the batter made no attempt to avoid, it is a dead ball, add a ball to count and keep batter at bat.
