    Please download and fill this form. Once filled out please submit the form via email to richard_vera@msn.com.

    - Copy of Birth Certificate
    - Picture of player (passport type)

    Failure to do so will delay the roster processing and you will not be carded.

    In order to play JAGS, the State of NJ requires all players to fill out this form. Please fill out this form and submit it with the US Club Form as well

  • JAGS Medical Release Form
    IMPORTANT: This form must be filled out by all players in order to play in the State of NJ and MUST BE NOTARIZED.

    This player’s guide was provided to us by Sam Snow, Director of Coaching and John Thomas, Assistant Director of Coaching from U.S. Youth Soccer. The guide is a great resource and is an easy to read summary that can help a player in obtaining a quick overview of the
